AB A  45-year-old  man  with  a  5-year  history  of  nonischemic  cardiomyopathy  with  an  ejection  fraction  of  30%  was  seen  in  routine  follow-up  in  the  heart  failure  (HF)  clinic.  He  had  only  mild  limitation  of  activity  with  New  York  Heart  Association  (NYHA)  Functional  Classification  I  to  II  symptoms.  He  had  been  on  a  stable  regimen  of  guideline-directed  medical  therapy  for  at  least  6  months.
